Instructions to Successfully Complete This NCPD Program
To receive a Certificate of Completion and be awarded NCPD contact hours for this program, follow the steps below, and complete and view the full content of the program including the program evaluation. If an assessment is included in the content, you will need to have 75% of the answer correct in order to pass the assessment. You will also have unlimited retakes on the assessment.

NASN will conduct an outcome assessment of a sample of participants at a later date. Please be advised that, if you complete this program, you may be asked to participate in this outcome assessment.

Step 1: Enroll in the Education Service Center Region 14 program.

Step 2: Complete and view the full content of the program.

Step 3: Complete the assessment and evaluation by September 7, 2023.

Step 4: Retrieve the certificate.  After completing all content items, you will earn a certificate. You can save or print it.  Certificates are also stored in your Learning Center Profile.

Program Overview

Focus on the Framework: Care Coordination is an interactive program is geared towards helping school nurses develop a deeper understanding of their role in chronic condition management, interdisciplinary team approaches, motivational interviewing, student health care plans, transitional planning and student self-empowerment. 

Learning Outcomes
As a result of participating in this educational activity, learners will be able to:

• Describe how the Framework for 21 st Century School Nursing Practice™ (the Framework) reflects a mindset for contemporary school nursing practice. 
• Articulate the practice components of care coordination as described in the Framework.
• Explain how care coordination is used to provide 21st century school nursing when addressing the health needs of students. 
• Apply the practice components of the care coordination principle to the steps of the nursing process. 
• Describe potential strategies and actions to help students and their families reach their identified health and academic goals through care coordination.

Intended Audience
This nursing continuing professional development program is developed to meet the educational needs in the area of knowledge and skill for registered nurses who are interested in child, adolescent health, community/public health, and school nursing.
